Salutem Care & Education in Eltham is seeking a Support Worker to join our Supported Living team. You will help adults with learning disabilities and autism to live independently, participate in daily activities, and build life skills in a caring, supportive environment.

You will work on person-centered plans, promote safety, and support health and daily living needs while collaborating with a compassionate team.

Qualifications

• Experience in care settings is preferred.
• Ability to support adults with learning disabilities and autism.
• Commitment to person-centered care and safety.

Responsabilités

• Implement person-centered support plans to promote independence and wellbeing.
• Develop flexible activity plans that encourage autonomy and growth.
• Support health, lifestyle, and daily living needs.
• Facilitate effective communication and maintain detailed records.
• Oversee medication processes and assist with self-administration.
• Maintain a clean, safe living environment and follow health and safety protocols.
