Job summary

Salutem Care and Education is seeking a dedicated Support Worker to provide person-centred support to adults with diverse needs at Thurston House in Newport Pagnall. The role emphasizes independence, dignity and holistic wellbeing within care plans and PBS guidelines.

You will work as part of a friendly team, assisting with daily living activities, personal care, medication management and community engagement while maintaining accurate records and collaborating with health professionals.

Responsibilities

• Provide person-centred support promoting independence and wellbeing.

• Maintain accurate records and documentation of daily notes and health records.
• Work with families and healthcare professionals to achieve positive outcomes.
