Live Connected is an exciting branch of the Connected Health group, providing quality care to adults with mental ill health, learning disabilities, dementia, and complex needs through supported living.

Key Duties and Responsibilities:

• Undertake daily living tasks as agreed with the service user, their family, and professionals involved.
• Work within the policies and procedures of Connected Living.
• Follow care and risk assessments specific to each service user's needs.
• Complete health and safety checks as per policies.
• Confidently work alone in line with lone worker risk assessments.
• Ensure safe medication delivery and ordering.
• Respect service users' preferences and promote choice and independence.
• Manage behaviors in the least restrictive way.
• Complete all documentation and provide high-quality handovers.
• Support the emotional and physical wellbeing of clients as part of a team.
• Develop and maintain professional relationships with service users and the multidisciplinary team.

Criteria:

• At least 6 months experience within Supported Living Services
• Valid Driving Licence
