Job summary

Partnership Care & Support Ltd in West London is seeking full-time and part-time Support Workers to join a compassionate team. You will provide care and assistance in line with individual care plans, supporting daily living activities and health needs.

Roles include day and night shifts, live-in and sleepover options, with a focus on person-centred care, dignity, and safety. DBS clearance and strong communication are required.

Responsibilities

• Provide support and assistance to individuals with disabilities or special needs in line with their care plan.
• Assist with daily living activities, such as personal hygiene, meal preparation, and medication management.
• Assist and encourage people to engage in everyday home, social, leisure and work activities, according to the individual’s choice.
• Support people with their health needs as required, e.g. taking medication and attending doctor’s appointments.
• Document and report any changes in behaviour or health status to the appropriate supervisor.
• Collaborate with a team of professionals to create and implement individualized care plans.
• Be able to provide excellent customer service.
• Knowledge and experience of CQC standards and CQC Compliance
• Undertake any other duties as appropriate.
• All staff are required to follow the Hygiene Code and all company policies and procedures related to it and the Health Act 2006.

Requirements

• High school diploma or equivalent
• Minimum 3-4 months experience working with individuals with disabilities or special needs
• Enhanced DBS

• Ability to follow instructions and adhere to safety protocols
• Flexibility to work various shifts, including evenings, weekends, and holidays
