Job summary

The Kingston Hub in Kingston upon Thames is recruiting a practitioner to manage and support a diverse range of casework under the Care Act 2014. You will screen referrals, assess needs and coordinate services with families and external health professionals.

You will ensure timely reviews, maintain robust records, and contribute to safeguarding and financial assessments while working within One Council policies. The role offers flexible working options and a supportive team environment.

Qualifications

• Experience delivering casework under the Care Act 2014.
• Ability to liaise with external health and care providers.
• Strong knowledge of safeguarding policies and data protection.

Responsibilities

• Manage referrals, conduct initial screening and provide information and advice.
• Undertake assessments and reassessments under Care Act 2014 and timelines.
• Liaise with GP, hospitals, care providers; work with families.
• Carry out financial information gathering for assessments.
• Contribute to duty system as required.
• Maintain accurate case records in systems.
