Job summary

This fixed-term post offers an exciting opportunity to work as a specialist Structured Clinical Management (SCM) practitioner within our South Community Mental Health Team. The successful applicant will benefit from being part of a small, supportive SCM team but also being situated within the wider Community Mental Health Team with all of the MDT working and interest that comes with this. This role is part of the increasing offer of Structured Clinical Management across the teams.

The post is open to mental health professionals (e.g. nurses, social workers occupational therapists or CAPs) who are passionate about offering high quality, evidence based care to a small caseload of service users with complex emotional difficulties. You will also have the opportunity to facilitate our group work alongside peer recovery workers and therapy colleagues.

We are keen to hear from candidates with training in and experience of delivering SCM but this is not essential. What is essential is that you are passionate about and experienced in working with this service user group and providing high quality services.

For internal candidates there is the possibility of seeking to undertake this fixed term post as a secondment opportunity.

Main duties of the job

The successful applicant will work with a small, capped caseload of service users participating in SCM but will also contribute to and be part of the wider CMHT MDT offering consultation, advice and support to our CMHT colleagues. We are looking for people who are compassionate and validating in their approach and are able to work thoughtfully and respectfully with service users, carers and colleagues.

We are excited about further developing our SCM offer and committed to supporting and investing in the staff team through psychologically minded supervision, training and team support.

Main roles and responsibilities include:-
  • To work as an SCM practitioner
  • To work within the SCM treatment team to deliver high quality evidence based treatment
  • To work alongside colleagues to facilitate the SCM groups
  • To manage a small SCM caseload
  • To receive regular SCM supervision
  • To offer supervision and SCM focused consultation to other team members.
  • Contribute to positive multi-disciplinary service development.
  • To assist in the development of patient related outcome measures and service user involvement

Structured Clinical Management is an evidence based treatment pathway for people with a diagnosis of personality disorder or who would identify with the difficulties associated with this diagnosis. The team is embedded within the wider CMHTs which are large MDT teams with a diverse case load. It feels exciting to be shaping something new, for our teams and for the people of Sheffield.

  • Carry out comprehensive assessments of an individuals health and social care needs and agree the most appropriate course of action
  • To carry an SCM caseload. To deliver SCM on a 1:1 basis with clients and to facilitate the SCM groups
  • To assess, evaluate and work collaboratively with service users to manage risk and provide advice to other professionals on collaborative safety work with clients.
  • To formulate a plan of care with service-users and carers based on identified need as informed by the SCM model.
  • Deliver SCM as an evidence based intervention as appropriate. To draw upon other treatment models as appropriate.
  • To contribute to the wider MDT by providing SCM informed guidance and consultations contributing to the service users formulation, diagnosis and treatment
  • To work positively with service users, carers and partner agencies.
  • To maintain computer held care records in an accurate and timely fashion in line with Trust policies and procedures.
  • To contribute to and help maintain the SCM evaluation outcome data
  • Undertake all work in a culturally sensitive manner.

Supervision:
  • Regularly review own SCM caseload with Lead SCM practitioner to promote efficacy and contribute to the overall management of the service.
  • Receive SCM supervision in line with the expectations of the treatment
  • Deliver supervision and facilitate PDR with more junior members of staff.
  • Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve level of competence and effective contribution.
