Job summary

Salisbury NHS Foundation Trust is seeking a motivated Band 4 Ward Based Therapy Assistant Practitioner with stroke and/or neuro experience to join the Stroke team on the ward at Salisbury Hospital. You will manage a varied caseload and contribute to rehabilitation plans in collaboration with the MDT.

The role includes leading discharge conversations, supervising group activities, and supporting patients to optimise quality of life with multidisciplinary support and supervision from senior staff.

Qualifications

• Experience in ward-based neuro/stroke rehabilitation.
• Ability to work with an MDT and discharge planning.

Due to increased funding for stroke intensity rehabilitation we are looking for a motivated individual who is committed to working as part of our neuro rehab team providing specialist stroke therapy to optimise quality of life.

We are looking for a Band 4 Ward Based Therapy Assistant Practitioner (TAP) with Stroke and/or Neuro experience to join the Stroke team. The role will be ward based on the stroke unit within Salisbury hospital working with a varied caseload including a range of neurological conditions within this specialised area.

Professional and specialist supervision is available from senior members of staff, and successful candidates will be working closely with all areas of the MDT. Our ideal candidate will be confident with appropriate NHS ward based experience, clearly able to demonstrate a desire to participate in all aspects of the role. The role will be ward based which will require the candidate to work with their allocated patient caseload to facilitate all aspects of their stroke rehabilitation. This will include progressing and following physical and cognitive rehabilitation plans that have been issued by their allocated physiotherapist or occupational therapist and some group work. There will also be an expectation to lead on basic discharge conversations.
