Dorset HealthCare University NHS Foundation Trust in Dorset is recruiting a Stroke and Neuro Nurse to join our experienced team on a part-time basis (21 hours per week). The role offers hybrid working across community, base and home-based care, with a west Dorset base at Forston Clinic and responsibilities spanning initial assessment through ongoing stroke and neurological care.

Qualifications

  • Experience in stroke and neuro care pathways is desirable.
  • Ability to work in a multidisciplinary team across community and hospital settings.

  • Manage a caseload and coordinate care across hospital discharge to home.
  • Offer supervision and mentorship to students and trainees.

Are you an established nurse looking to be part of an experienced team? Our Dorset Stroke and Neuro Service would love to hear from you where ideas are shared, and visions are supported.

You will provide specialist advice with a high-quality person-centred approach to care delivery which always considers people’s safety, privacy and dignity. Our integrated community stroke and neuro team is AHP lead and provide various pathways including supported self-management, advice with signposting, intensive rehabilitation (ESD), complex management and reviews. You will be working in a multidisciplinary team including social work, neuropsychology, rotations across the acute and community pathway and opportunities for advanced practice. You will have access to a wide range of specialists with many years of experience who can help to increase your opportunities for learning and development of skills.

The specialist nurse post is a part time role of 21 hours. Our service is a 6-day service. We promote hybrid working with mixed caseload of community, base and working from home;

West Dorset (base at Forston Clinic) covering Weymouth & Portland, Lyme Regis, Purbecks and Dorchester.

To provide a high quality person centred approach to care delivery which always considers people's safety, privacy and dignity.

To provide specialist care and interventions, assessments, treatments and managing patients on a caseload, as part of the wider multi-disciplinary care team and along the stroke and neuro pathway from hospital discharge for as long as needed.

To provide specialist advice to others regarding the management and care of patients/ service users.

To devise specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.

To plan, implement and review health improvement programmes in a range of settings.

Provide supervision, mentorship, training and clinical support to students and trained practitioners.

To coordinate and oversee the provision of nursing care for stroke and neuro patients in the North and West Dorset localities by identifying and managing a caseload of patients diagnosed with having a stroke or a neurological condition.

To provide nursing support for patients within integrated community Stroke and Neuro Service.

To be actively involved in the development of community, stroke and neuro rehabilitation services across Dorset.

To develop staff skills and knowledge of stroke and neurological conditions across Dorset.
