Job summary
The Rotherham NHS Foundation Trust is seeking an Assistant Practitioner (Band 4) within our Stroke Therapy Team in Rotherham. This full-time permanent role involves autonomous caseload management, contributing to MDT stroke care, and delivering high-quality rehab to stroke survivors.
You will work across hospital and community settings, support 6-month reviews, participate in service improvement, and travel around Rotherham with a suitable vehicle. Applicants recently qualified AHPs are welcome.
Qualifications
- Registered allied health professional (AHP) or recently qualified
- Ability to travel across Rotherham with a vehicle
- Willingness to undertake formal competency frameworks (e.g. SSEF)
- Experience in stroke rehabilitation and community settings
- Knowledge of Health & Safety and Clinical Governance
Responsibilities
- Manage and prioritise a caseload of patients under direction
- Contribute to MDT pathway across hospital and community settings
- Coordinate 6-month reviews clinics and support national targets (SSNAP)
- Support service improvement with Team Leaders and Clinical Lead
