Job summary

East Coast Community Healthcare CIC in Great Yarmouth is developing an Integrated Community Stroke Service (ICSS) to coordinate stroke pathways and enable earlier discharge. You will join a MDT including nurses and therapists to deliver community rehabilitation and triage patients.

The role involves assisting with assessments, carrying a caseload of non-complex patients, and supporting treatment plans, with both face-to-face and telerehabilitation sessions as part of an integrated stroke team.

Qualifications

• Experience in community stroke rehabilitation or similar setting.
• Ability to work with the MDT to plan and deliver care.
• Willingness to undertake initial assessments and triage.

Responsibilities

• Assist therapists and nurses in delivering the Stroke Integrated Community Stroke Service.
• Undertake tasks within the non-complex patient remit in a community setting.
• Carry a caseload, devise and implement treatment programmes with supervision.
• Support assessment of complex cases and ongoing management as part of the MDT.
• Contribute to face-to-face and telerehab sessions and develop self-management plans.
