Job summary

East Coast Community Healthcare CIC in Great Yarmouth is developing an Integrated Community Stroke Service (ICSS) to coordinate stroke pathways and enable earlier discharge. You will join a MDT including nurses and therapists to deliver community rehabilitation and triage patients.

The role involves assisting with assessments, carrying a caseload of non-complex patients, and supporting treatment plans, with both face-to-face and telerehabilitation sessions as part of an integrated stroke team.

Qualifications

• Experience in community stroke rehabilitation or similar setting.
• Ability to work with the MDT to plan and deliver care.
• Willingness to undertake initial assessments and triage.

Responsibilities

Job description

We are developing an Integrated Community Stroke Service (ICSS) within Great Yarmouth & Waveney, bringing together community stroke pathways into a more coordinated, responsive and patient-centred model of care. As part of the wider left shift ambition, this service will support earlier discharge from hospital and improve outcomes through timely, multidisciplinary rehabilitation in the community. Our Year 1 focus is on building a more responsive and integrated local service, increased MDT capacity and improved continuity of care across the stroke pathway. You will be joining a service at a key point of development, with the opportunity to shape how care is delivered and contribute to a more integrated, forward-thinking model of community stroke rehabilitation.

Working alongside the other MDT members including nurses, physiotherapists, speech and language therapists and occupational therapists your duties will include:

• To assist qualified therapists and nurses in the delivery of the Stroke Integrated Community Stroke Service
• To work without direct supervision, in a community setting, undertaking tasks that fall within the remit of the non-complex patient. This can include frontline initial assessments to triage stroke patients
• To carry a caseload of non-complex patients, devising and implementing treatment programmes with reference to a qualified therapist or nurse
• Assisting qualified Nurses, Occupational therapists, Physiotherapists and Speech and language therapist with the assessment of complex cases and ongoing management
• To work as part of an integrated Stroke MDT, following evidence based practice. This will include a combination of face-to-face and telerehab sessions, devising therapy programmes carried out by yourself or an Assistant Practitioner, writing self management plans for patients and signposting to community-based communication and support groups (such as those provided by voluntary organisations).
