Job summary

East Coast Community Healthcare is developing an Integrated Community Stroke Service in Great Yarmouth & Waveney. We seek a skilled nurse to lead the community stroke nursing team and deliver high quality specialist rehabilitation to help stroke patients regain independence and maximize recovery potential.

You will contribute to post-stroke reviews, provide essential nursing assessments, medication reviews, and liaise with other services to coordinate care in a forward-thinking community

Responsibilities

• Lead a community stroke nursing team within the Integrated Community Stroke Service, providing high quality specialist rehabilitation to patients following a stroke.
• Support post-stroke reviews provided by other services for all stroke survivors within the locality to identify difficulties and needs.
• Provide specialist nursing within the service including assessment, liaison and treatment of stroke patients.
• Provide basic medication reviews and clinical observations and liaise with other professionals as required.
• Assess and prescribe basic pressure care equipment and signpost to other involved services.
• Complete mood screen assessments and refer onwards to relevant agencies as needed.
• Provide basic continence assessments and liaise with specialist teams for input when required.

Job description

We are developing an Integrated Community Stroke Service (ICSS) within Great Yarmouth & Waveney, bringing together community stroke pathways into a more coordinated, responsive and patient-centred model of care. As part of the wider left shift ambition, this service will support earlier discharge from hospital and improve outcomes through timely, multidisciplinary rehabilitation in the community. Our Year 1 focus is on building a more responsive and integrated local service, increased MDT capacity and improved continuity of care across the stroke pathway. You will be joining a service at a key point of development, with the opportunity to shape how care is delivered and contribute to a more integrated, forward-thinking model of community stroke rehabilitation.,

• To lead a community stroke nursing team within the East Coast Community Healthcare Integrated Community Stroke Service, providing high quality specialist rehabilitation to patients following a stroke in order to allow patients to achieve their optimal level of independence and maximum rehabilitation potential.
• To support post stroke reviews provided by other services for all stroke survivors within the Great Yarmouth and Waveney locality to identify difficulties and needs, provide information, advice and support for patients and their family and/or carers.
• To provide specialist nursing within the Integrated Community Stroke Service consisting of assessment, liaison and treatment of patients following stroke.
• To provide basic medication reviews, clinical observations and liaise with professionals as required.
• To assess and prescribe basic pressure care equipment and signpost to other services requiring to be involved.
• Complete basic mood screen assessments, have an awareness of the Psychological Step Care Model and carry out levels 1+2. Refer onwards to other agencies for further input if required.
• To provide basic continence assessments, provide advice and refer to specialist team for input if required.
