Responsibilities

• Lead and coordinate transformation projects and service improvement initiatives across neighbourhoods and the wider Health & Care Partnership.
• Translate strategic priorities into practical delivery plans, ensuring programmes are delivered on time and within scope.
• Provide programme and project management expertise, including governance, risk management, performance monitoring and benefits realisation.
• Develop relationships with health, social care, local authorities, primary care and community partners to support collaborative delivery.
