Job summary

West Hertfordshire Teaching Hospitals NHS Trust is seeking two Band 8a Neighbourhood Delivery Managers to join the South and West Hertfordshire Health and Care Partnership during a period of transformation.

You will lead neighbourhood transformation delivery, coordinate programmes across NHS bodies, local authorities, primary care and voluntary sectors to achieve integrated services and measurable outcomes.

Responsibilities

• Lead and coordinate the delivery of neighbourhood transformation priorities across South and West Hertfordshire.
• Support the implementation of programmes and projects that improve health and care outcomes for local residents.
• Work collaboratively with NHS organisations, local authorities, primary care, voluntary sector partners and community groups to deliver integrated services.
• Translate strategic ambitions into practical delivery plans and measurable outcomes.
• Monitor progress, manage risks, and ensure benefits from transformation programmes are realised.
• Support neighbourhood leadership teams in driving innovation, service improvement and new models of care.
• Facilitate effective communication and engagement across a wide range of stakeholders.
• Contribute to Health and Care Partnership-wide transformation programmes across planned and unplanned care services.
