Job summary

HCRG Care Group is seeking an experienced and dynamic Special School Nursing Partnership Lead to drive Surrey’s countywide service redesign, moving to a Hub and Spoke model. You will oversee planning, delivery, governance and risk management across health, education and social care, ensuring contractual and population health needs are met.

This fixed-term role until July 2027 offers Band 7 AfC salary with NHS Pension, access to learning and development, and participation in an innovative

Qualifications

• Degree-level qualification or equivalent in healthcare, nursing, education, service improvement, project management or related field.
• Experience leading complex service redesigns and change programmes.
• Experience across health, education and social care systems.

Responsibilities

• Lead Surrey's Special School Nursing redesign and Hub and Spoke model implementation.
• Manage programme delivery, milestones, quality standards and contractual requirements.
• Build relationships with Surrey County Council, ICB, schools and families.
• Lead workforce engagement and change management activities.
• Review pathways and processes to improve accessibility and efficiency.
• Oversee governance, risk management and reporting.
• Use data and quality improvement methods to measure benefits.
• Produce reports for senior leaders and partners.
• Identify opportunities to improve service delivery and outcomes for children.
