Job summary

HCRG Care Group is seeking an experienced Special School Nursing Partnership Lead to drive Surrey’s countywide service redesign from a traditional model to a Hub and Spoke approach. You will plan, implement, govern the programme and ensure delivery meets contractual and population health needs across Surrey.

You will collaborate with Surrey County Council, the ICB, special schools and families, lead workforce engagement and change management, and use data to measure benefits and assure quality

Qualifications

• Degree-level qualification or equivalent experience in healthcare, nursing, education, or project management.
• Experience leading complex service redesign and change programmes.
• Experience working across health, education and social care systems.
• Strong stakeholder engagement and partnership skills.
• Experience of workforce planning and organisational change.

Responsibilities

• Lead the implementation of Surrey's Special School Nursing Service redesign to Hub and Spoke model.
• Manage programme delivery with milestones, quality standards, contractual requirements and outcomes.
• Build relationships with Surrey County Council, ICB, special schools and families.
• Lead workforce engagement and change management during transformation.
• Use data and quality improvement methods to drive decisions and measure benefits.
• Produce reports for senior leaders and commissioners with clear accountability.
