An exciting opportunity has become available to join the Planned Community Nursing team across Doncaster. The community nursing service is located in the Doncaster Care Group, with an integrated Physical and Mental health model developing through the community services, across four localities. The service is part of a forward thinking Organisation, which supports development and innovation at all levels to enhance the quality of patient care and service development.
Applications are invited from forward thinking, highly motivated clinicians who are able to demonstrate clinical knowledge and skills with evidence of leadership experience.
As the SPQDN/ District Nurse Sister / Charge Nurse for your team you will be responsible for the Management of your team and your caseload. You will holistically manage the patient care on your caseload through a Case Management approach the aim of which is to support patients to remain in
their own homes, promoting self-care and independence.
You will need excellent interpersonal and communication skills, the ability to negotiate, priorities, manage time effectively, work on own initiative and the ability to work under pressure in a rapidly changing environment.
You will contribute towards the ongoing improvement of community services in Doncaster through participation in audit and the development of best practice.
Furthermore you would have a key role in continuing education through staff appraisals and training and will contribute to the creation of a clinical area that encourages practice development, positive learning and disseminate information on developments.
