Job summary

Dorset HealthCare is seeking a Clinical Practice Specialist Practitioner to work within the Home Treatment Team in Dorchester. The post supports adults with acute mental health difficulties, preventing admission where possible.

You will plan, implement and review care with a multidisciplinary team and provide supervision and training to learners and staff. The role is full-time with part-time consideration, requires a UK driving licence and travel by car, and operates 09:00–22:00 on a rota basis

Qualifications

• Registered professional in one of the following: Mental Health Nurse, Occupational Therapist or Social Worker.
• Current registration with the appropriate regulatory body (NMC/HCPC).
• Experience in mental health care and home treatment settings is desirable.

Job description

Are you a highly motivated, reliable and dynamic registered Mental Health Nurse, Occupational Therapist or Social Worker with experience of working with people suffering with mental health? An opportunity has arisen within the Home Treatment Team. The Home Treatment Team works closely with the community mental health teams and the inpatient units to provide a period of intervention to a range of people experiencing acute mental health difficulties to avoid the need for hospital admission or to reduce the length of stay. As a Clinical Practice Specialist Practitioner, you will provide specialist care and interventions, assessments, treatments and managing patients on a caseload, as part of the wider multidisciplinary care team. You will plan, implement, and review health improvement programmes in a range of settings. Provide supervision, mentorship, training and clinical support to students and trained practitioners. You will receive supervision, support, and training to develop your skills within this role. This post is full-time hours however part-time hours would be considered. The Home Treatment team is a 7 day a week service including public holidays, operating from 09:00 - 22:00. We work on rota basis 09:00 - 17:00 / 14:00 - 22:00. We would consider a 9-5 duty role worked over the 5-day working week Monday to Friday. Your work base will be Forston Clinic, Dorchester DT2 9TB. Driving is essential for this role. You will be required to have a full and valid UK driving licence and to travel using own vehicle on Trust business. As part of the team assess, plan, implement and evaluate specialist treatment and care to people on an allocated caseload; promoting independence and autonomy; working within a multi-disciplinary team. You will be required to advance your own clinical knowledge, skill and competence based on current evidence through advanced educational programmes. Be able to recognise, assess, and manage risk across the immediate and wider working environment and make appropriate decision autonomously ensuring statutory requirements are met. Offer your support in the management of change through strategic thinking, use of negotiating skills, self-awareness and communication. Understand and evaluate care, taking appropriate action leading to improvement in quality standards through clinical audit, root cause analysis and dealing with complaints.
