Job summary

Somerset NHS Foundation Trust is seeking a Mental Health Practitioner to support adults with severe mental health needs in the community. You’ll perform comprehensive assessments, co-develop recovery and risk plans, and deliver psychosocial interventions to promote independence and wellbeing.

You will collaborate with health, social care and voluntary sector partners, contributing to service development and governance. Driving is required for community visits and access to transport is essential.

Qualifications

• Car driver with access to transport for work purposes
• Community mental health experience would be advantageous

Responsibilities

• Manage a varied caseload of adults with complex mental health needs within the community.
• Complete comprehensive mental health and risk assessments using a strengths-based, recovery-focused approach.
• Develop, implement and review personalised recovery and risk management plans.
• Deliver evidence-based psychosocial interventions that promote independence, wellbeing and resilience.
• Build effective therapeutic relationships with service users, families and carers.
• Work collaboratively with colleagues across health, social care, primary care and voluntary sector organisations.
• Support individuals through periods of crisis, relapse and recovery while promoting positive outcomes.
• Act as an advocate for service users, ensuring their voice remains central to care planning and decision-making.
• Contribute to clinical leadership within the team through supervision, mentoring and supporting less experienced colleagues where appropriate.
• Participate in service development, quality improvement and clinical governance initiatives.
