Are you passionate about supporting patients at home and committed to improving their quality of life? Ready for a new challenge? Join our friendly team as a Band 6 Specialist Physiotherapist to deliver holistic physiotherapy interventions, prevent hospital admissions, and support discharges.

Our team covers Selby and surrounding villages, including Physiotherapists, Occupational Therapists, Nurses, and Support Workers, working alongside Primary Care to provide Urgent Care Response and short-term reablement/rehabilitation with clear goals.

This role involves using health coaching to engage patients and families effectively.

Transport and travel are essential; access to a vehicle and a full UK driving licence are required.

Main duties of the job
  • Conduct specialist physiotherapy assessments and manage patients referred to the service.
  • Determine diagnoses and deliver appropriate interventions.
  • Support and train junior staff and students.
  • Provide expert advice within the department and Trust.
  • Maintain accurate records in line with legislation and policies.
  • Support colleagues with patient care.
  • Manage a defined service area independently.
  • Lead audits and projects to improve practice and involve users in service development.
