Job summary

Qualifications

This post requires full registration with a license to practice and at least 2 years post-registration clinical experience.

Responsibilities

Collaborate with the MDT including palliative care Clinical Nurse Specialists and End of Life Care Nurses.
Review patients referred to the Specialist Palliative Care Team on wards including ED.
Electronic prescribing of medications for symptom management for SPCT caseload.
Lead and/or attend family meetings as required.
Escalate for senior support/review when needed.
Liaise with medical and surgical teams within LDH and provide SPC advice.
Care for patients in the palliative virtual ward, including home visits as appropriate.
Act as first point of medical advice for community CNSs for PVW patients.
Attend the weekly SPC MDT and morning pan-Bedfordshire SPC huddle.
