Job summary

Bedfordshire Hospitals NHS Foundation Trust in Bedfordshire is seeking a Specialist Palliative Care Team clinician to work within the SPCT at LDH, contributing to MDT-based patient assessments and symptom management.

You will lead family meetings, coordinate referrals, and support PVW community care, while maintaining clear clinical notes and providing expert SPC guidance across hospital and community settings.

Responsibilities

• Collaborate with MDT members and palliative care CNSs to review patients on SPCT caseloads across wards, including ED.
• Perform new patient assessments and ongoing reviews for SPCT caseload.
• Lead and/or attend family meetings as required and manage complex communications.
• Escalate for senior review when necessary and liaise with medical and surgical teams within LDH.
• Provide care to PVW patients including home visits per SOP, act as first point of call for community CNSs.
