Job summary

Active Care Group’s Blackheath Brain Injury Rehabilitation Centre is seeking an HCPC-registered Occupational Therapist to deliver neurorehabilitation interventions, manage a complex caseload and coordinate patient journeys within a multidisciplinary team.

You will contribute to diagnosis, planning and safe discharges, while maintaining accurate records and supporting junior staff. DBS checks and commitment to patient-centred outcomes are essential.

Qualifications

• Experience in neurorehabilitation settings.
• Able to manage a complex caseload and coordinate patient pathways.
• Competent in maintaining high quality clinical notes and MDT communication.

Responsibilities

• Deliver safe, evidence-based occupational therapy interventions grounded in neuroplasticity and motor relearning.
• Act as Key Worker to coordinate rehabilitation journey for patients with complex needs.
• Contribute to diagnosis, problem formulation and rehabilitation planning within MDT; support safe discharges and equipment provision.

Blackheath Brain Injury Rehabilitation Centre, part of Active Care Group, is a specialist neurorehabilitation service with 39 beds, delivering high-quality rehabilitation for adults with acquired brain injury and complex neurological conditions. We support individuals with Level 1 and Level 2 rehabilitation needs, including those with a combination of physical, cognitive, behavioural, and psychosocial challenges. Rehabilitation is delivered through a coordinated multidisciplinary team (MDT) approach, with a strong focus on evidence-based practice, structured care pathways, and goal-oriented, patient-centred outcomes. We pride ourselves on a collaborative and supportive team culture, underpinned by strong clinical governance and a commitment to continuous service improvement—ensuring patients receive safe, effective rehabilitation that maximises independence and quality of life.

We are currently looking for an experienced Occupational Therapist to join our team and make a real difference in the lives of our patients.

The role:

• Deliver safe, innovative, evidence-based OT interventions grounded in neuroplasticity and motor re-learning principles
• Manage and prioritise a complex caseload, acting as a Key Worker for patients to coordinate their rehabilitation journey
• Contribute to diagnosis, problem formulation, and rehabilitation planning as part of an MDT Plan and support safe discharges, including prescribing and providing equipment where needed
• Maintain high standards of clinical records and ensure timely, effective communication with patients, families, the MDT, and external agencies
• Supervise junior staff, assistants, and students and contribute to in-service training
• Take part in service developments and may occasionally provide cover at our nearby Chislehurst site

Person Specification:

• HCPC-registered Occupational Therapist
• Experience working within a neurorehabilitation setting
• Confident managing a complex caseload and delivering patient-centred interventions
• Motivated, collaborative, and committed to delivering meaningful outcomes

Successful candidates will be required to undergo an Enhanced DBS
