Job overview

This is an ideal role for a pharmacist wanting to work in Nutrition and Intestinal Failure in a nationally designated specialist centre. This role would enable you to develop your clinical knowledge and skills and to work with a very motivated wider multidisciplinary team for both inpatients and homecare patients.

Main duties of the job

The post-holder will undertake a number of duties, and in particular duties focussed on clinical nutrition:

  • Advise on medicines management for nutrition support patients.
  • Actively participate as a member of daily inpatient PN ward rounds that take place on different hospital sites (primarily at the Churchill and John Radcliffe sites).
  • Liaise with appropriate nursing and/or medical staff for vascular access device insertion if needed.
  • Act as a non-medical prescriber for PN patients.
  • Attend and support nutrition MDT meetings, for both inpatients and outpatients.
  • Offer advice on the management of home PN patients to the nutrition team and other hospital staff, as well as to patients directly.
  • Liaise and negotiate service provision with external providers e.g. for homecare PN.
  • Report and investigate adverse incidents related to nutrition support.
  • To assist the Lead and Deputy Team Pharmacists in financial and commissioning reporting for the intestinal failure service.
  • Participate in SHIFNET and/or BPNG and/or other local/national group related to PN provision.
  • Provide teaching regarding PN to pharmacy, nursing, medical staff and other allied healthcare professionals (AHPs).
