Job summary

Humber Teaching NHS Foundation Trust is seeking a Band 6 Specialist Practitioner for the Crisis Intervention Team for Older People (CITOP) to lead on innovative care pathways and support 24/7 crisis intervention across Hull and East Riding.

You will apply expertise in older adult mental health to triage, assess and deliver crisis interventions, while supervising and developing staff within a multidisciplinary team.

Qualifications

• Diploma or Degree in Professional qualification equivalent to RMN, Registered Social worker, OT or equivalent AHP
• Registration with the HCPC/NMC or equivalent
• Evidence of CPD/formal qualifications at an advanced level e.g. degree or postgraduate level
• Knowledge/experience of working in the relevant service area or similar
• Evidence of development of specialist practice nursing skills and impact on practice change

Responsibilities

• Utilise specialist knowledge of Older Adult Mental Health to provide crisis triage and assessments, planning and delivering a variety of crisis interventions
• Provide Home Treatment & Acute Community Service interventions as an alternative to hospital admission or to support timely admission to and discharge from inpatient settings

• Work within a MDT ensuring quality standards and patient safety within clinical governance framework

Job summary

An exciting opportunity has arisen for the role of a band 6 Specialist Practitioner to the Crisis Intervention Team for Older People (CITOP) due to the expansion of the team's current role. The CITOP team supports adults predominately aged over 65 in the Hull and East Riding area with both functional and organic illness.

The CITOP team provide 24-hour Crisis intervention and Home treatment for older adults across Hull and East Riding of Yorkshire. CITOP will be provide four Community Crisis Beds for older adults as an alternative to inpatient care and The Acute Community Service will be extended to provide a seven-day service.

Due to the development of the CITOP service we are looking for professionals who are keen to lead on innovation, developing robust care pathways and enhance the patient and carer experience.

We are seeking individuals who are motivated, enthusiastic and have experience of working with people who have complex needs and enduring mental illness. This role requires good organisational skills, being able to demonstrate an ability to prioritise and delegate work effectively, and an interest or specialism in urgent and emergency mental health care, providing expert clinical advice and support and provide leadership and guidance on shift.

Main duties of the job

• The main features of this role will include the utilisation of the specialist knowledge of Older Adult Mental Health care to provide crisis triage and assessments , planning and delivering a variety of Crisis interventions, whilst reviewing care and planning for discharge within the community setting across Hull and East Yorkshire.
• You will provide Home Treatment & Acute Community Service interventions as an alternative to hospital admission or support timely admission to and discharge from inpatient settings. You will be required to collect/collate information and attend formulations for patients working in collaboration with others to ensure the best care for service users. You will be working within a Multi-Disciplinary Team maintaining quality standards and patient safety, within a framework of clinical governance.
• You will have the opportunity to develop leadership skills, providing supervision to healthcare staff. You will lead and participate in the development of staff through appraisal and clinical supervision.
• The applicant will be expected to have knowledge in mental health conditions, including Dementia and experience in this area would be an advantage, additional training and support will be given.
• We are a 24/7 service therefore patterns of duties include days, evenings, weekend's, nights and bank holidays

Qualifications and Knowledge

Essential

• Use of specialist knowledge and experience acquired in a relevant setting underpinned by theory
• A broad understanding of relevant policy, legislation drivers and their application to clinical practice and service
