Job summary

CYP Practitioner position within HPFT in South Hertfordshire offers a full-time permanent role (37.5 hours) focused on delivering high-quality mental health care for children and young people. You will participate in referrals, assessments and delivery of evidence-based interventions, while coordinating care and contributing to safeguarding.

The role includes consultation and supervision of colleagues and students, with a strong MDT component.

Qualifications

• Must hold a professional qualification with relevant experience.
• Experience in risk management and safeguarding children and vulnerable adults.
• Ability to deliver evidence-based interventions and coordinate care.
• Experience in supervising colleagues and contributing to multidisciplinary teams.
• Participation in supervision, appraisal and professional development.

Responsibilities

• Develop and provide highly specialist evidence-based interventions for service users.
• Deliver services through clinic-based and outreach models.
• Provide nursing consultation to colleagues and external agencies.
• Care coordinate a caseload delivering highly specialist assessments and integrated care.
• Lead nursing development and supervise nursing and other disciplines.
• Participate in supervision, appraisal and professional development.
• Chair or attend MDTs for clinical sign off.
• Collaborate with HPFT and external agencies to improve early identification and management.
• Lead service innovation and development to ensure safe, effective care.
