Job summary

HCRG Care Group is seeking a compassionate Specialist Community Learning Disability Nurse (RNLD) to join our Swindon team on a 6-month fixed-term maternity cover contract. You will deliver specialist assessment, intervention and care coordination for adults with learning disabilities, in a multidisciplinary team.

Qualifications

• RNLD or RMN registration with area of practice in learning disability.
• Experience in community services and/or learning disability and autism services.
• Strong assessment, clinical decision-making and care planning skills.

Responsibilities

• Undertake comprehensive assessments and risk assessments.
• Provide specialist interventions and ongoing support for adults with learning disabilities.
• Manage a defined caseload with safe, person-centred care.
• Assess service user needs and determine appropriate care pathways.
• Lead and contribute to multidisciplinary meetings and complex case discussions.
• Collaborate with a range of professionals to deliver high-quality care.
• Develop partnerships to support positive outcomes and service access.
• Contribute to service development and quality improvement.
