Job summary

HCRG Care Group is seeking a Specialist Community Learning Disability Nurse (RNLD) to join the Swindon team. You will assess, plan and coordinate care for adults with learning disabilities, working within a multidisciplinary service to promote independence and wellbeing.

You will deliver specialist interventions, support families and carers, and lead care planning across complex cases while ensuring high-quality, accessible services within the Learning Disability Team.

Qualifications

• Registered Nurse (RNLD or RMN) with professional registration.
• Experience in community services or learning disability settings.
• Experience supporting individuals with complex needs and challenging behaviours.

Responsibilities

• Undertake comprehensive assessments and risk assessments to identify needs.
• Provide specialist interventions and ongoing support to individuals and families.
• Manage a defined caseload delivering safe, effective care.
• Lead MDT meetings and contribute to complex case discussions.
• Collaborate with professionals to deliver person-centred care.
