Job summary

HCRG Care Group is seeking a Health Visitor to lead the Healthy Child Programme in your locality, delivering targeted interventions for children and families while upholding safeguarding responsibilities.

Based in England with flexible hybrid working, you will manage a defined caseload within a teams environment, support learners, and drive service improvement through evidence-based practice.

Qualifications

• SCPHN Health Visiting qualification is essential.
• Current NMC registration is required.
• Experience in health visiting teams is essential.
• Strong safeguarding knowledge is required.
• Ability to lead and motivate a multidisciplinary team.
• Excellent communication and partnership-working skills.
• Confidence working autonomously with a caseload.
• IT literacy and use of electronic health records.

Responsibilities

• Lead the delivery of the Healthy Child Programme within your locality.
• Assess health needs and implement targeted, outcome-focused interventions for children and families.
• Take responsibility for safeguarding and outcomes of children on child protection plans.
• Collaborate with multi-agency partners to support vulnerable families.
• Promote health and wellbeing through early intervention, education, and signposting.
• Manage and co-ordinate a defined caseload, working autonomously and within a team.
• Delegate and support skill mix teams to deliver effective care.
• Contribute to multi-agency assessments and act as Lead Professional where appropriate.
• Drive service improvement through leadership, innovation, and evidence-based practice.
• Support and develop colleagues, learners, and junior staff through supervision and training.

Job description

This role is central to improving outcomes for children, reducing health inequalities, and ensuring safeguarding across the community. You will work as part of a skill-mixed team, leading public health initiatives and delivering the Healthy Child Programme across your locality. HCRG Care Group is a flexible-first employer, offering agile and hybrid working, with a local office base to support team collaboration.

Main Responsibilities As a Health Visitor, you will:

• Lead the delivery of the Healthy Child Programme (Universal Partnership Plus) within your locality
• Assess health needs and implement targeted, outcome-focused interventions for children and families
• Take responsibility for safeguarding and outcomes of children on child protection plans
• Work collaboratively with multi-agency partners to support vulnerable families
• Promote health and wellbeing through early intervention, education, and signposting
• Manage and co-ordinate a defined caseload, working both autonomously and within a wider team
• Delegate and support skill mix teams to deliver effective care
• Contribute to multi‑agency assessments and act as Lead Professional where appropriate
• Drive service improvement through leadership, innovation, and evidence-based practice
• Support and develop colleagues, learners, and junior staff through supervision and training

Leadership & Service Development You will also:

• Provide leadership across the health visiting team, including caseload management and allocation
• Lead on public health initiatives within your locality
• Contribute to service development, quality improvement, and pathway design
• Act as a champion for change and innovation within the service
• Support staff development through appraisals, supervision, and training planning
• Use evidence-based practice to maintain high standards of care

Qualifications

• Specialist Community Public Health Nursing (SCPHN - Health Visiting) qualification
• Current NMC registration (third part of the register)
• Experience working within health visiting teams
• Strong safeguarding knowledge and experience supporting vulnerable families
• Ability to lead, motivate, and support a multidisciplinary team
• Excellent communication and partnership working skills
• Confidence working autonomously and managing a caseload
• IT literacy and experience using electronic health records

Desirable

• Leadership or management qualification
• Experience in service development or quality improvement
• Experience working with commissioners or external partners
