Lincolnshire Partnership NHS Foundation Trust is seeking a clinical pharmacist to provide patient-facing pharmaceutical care on inpatient wards and in community teams, with a focus on mental health.

The role may start at Band 6 with a development pathway to Band 7, supported by a PG certificate and diploma competency framework. We offer leadership development, flexible working, and ongoing training opportunities.

Qualifications

  • Registered pharmacist with current GPhC registration.
  • Clinical and patient-facing focus.
  • Experience in mental health pharmacy desirable.

Responsibilities

  • Take local responsibility for pharmacist services to allocated clinical areas in line with rotas.
  • Review stock lists regularly, considering usage, costs and prescribing trends.
  • Provide pharmacist cover to other clinical areas as required.
  • Review prescriptions and advise on medicines, side-effects, dosages, administration, supply, storage and destruction.
  • Ensure Mental Health Act paperwork (T2, T3 and CTOs) is accurate and follow up anomalies.
  • Contribute to multidisciplinary team meetings and influence treatment plans; promote medicines management and evidence-based prescribing.

Job description

Are you a clinical pharmacist with a passion for delivering outstanding care that brings the 6 Cs (care, compassion, commitment, courage, communication and competence) to life? If so, we want to hear from you. We are seeking to appoint a highly motivated pharmacist with a clinical and patient-facing focus to join our established and friendly Pharmacy team.

You will be responsible for delivering a clinical service on our inpatient wards and within our community teams. An excellent leadership and clinical development package will be on offer for our successful candidates as we aim to invest in our staff to deliver high standards of care to our patients.

This is a clinical role and a great opportunity for pharmacists from a variety of backgrounds to gain experience in mental health pharmacy.

Depending on experience, this post will either be a developmental post (band 6 role which will develop into a B7 as the postholder will be supported through a PG certificate and diploma competency framework) or direct into a band 7 role.

  • Take local responsibility for pharmacist services to allocated clinical areas in line with rotas.
  • Review any relevant stock lists regularly, taking into consideration usage, service resilience, overall costs and prescribing trends.
  • Provide Pharmacist Cover To Other Clinical Areas As Required.
  • Review prescriptions and information relating to individual patients and provide relevant advice about medicines, possible side-effects, dosages, administration, supply, storage and destruction.
  • Ensure Mental Health Act paperwork i.e. T2's, T3'S and CTO's is accurate and follow up where there are anomalies in line with departmental procedures.
  • Contribute to multi-disciplinary working within the allocated clinical area including participation in meetings, influencing the formulation and review of treatment plans and ensuring safe and effective pharmaceutical treatment appropriate for the individuals or groups. Promote best practice in medicines management, medicines optimisation and evidence-based prescribing.
