Sheffield Health Partnership University NHS Foundation Pharmacy Department is recruiting a highly motivated pharmacist to join a forward-thinking team delivering clinical pharmacy services across the Trust. You will support medicines use, conduct medicines reviews and provide medicines optimisation advice.

The role includes on-call duties, weekend and late rotas, and travel to locations across the city, with opportunities for professional development and post-registration qualifications.

Responsibilities

  • Support the use of medicines across the Trust to achieve best outcomes.
  • Apply clinical skills in medicines management for mental health.
  • Prevent and reduce medicines-related harm.
  • Provide high-level clinical pharmacy services to allocated teams.
  • Undertake medicines reviews and advise on medicines optimisation.
  • Use electronic prescribing and medicines administration systems effectively.
  • Engage in CPD and pursue post-registration qualifications as needed.
  • Participate in departmental supervision and peer support.
  • Manage day-to-day running of the clinical trials service.
  • Work flexibly and travel to sites across the city.
  • Contribute to on-call pharmacist service and weekend/late rotas.

  • 1. To support the use of medicines across the Trust to achieve best outcomes for service users in accordance with best practice.
  • 2. To apply clinical skills and expertise in the management of medicines in mental health in accordance with best practice.
  • 3. To prevent and reduce medicines related harm.
  • 4. To provide a high level of clinical pharmacy services to allocated teams.
  • 5. To undertake medications reviews and provide advice on medicines optimisation.
  • 6. Competently use electronic based notes and prescribing systems.
  • 7. Participate in systems to ensure continuous personal professional development, including any agreed post-registration qualifications. Work towards membership of the College of Mental Health Pharmacy (or equivalent).
  • 8. Participate in departmental systems of clinical supervision and peer support.
  • 9. Responsible for the day to day running of clinical trials service, including pharmacy trial specific procedures.
  • 10. To work flexibly and travel to various locations across the city.
  • 11. To participate in On-call pharmacist service in accordance with local procedure.
  • 12. Participate in weekend, bank holiday and late working rotas in accordance with local procedure.

The Pharmacy Department is a forward thinking and progressive department seeking to expand our current team as we develop a patient focused and modern hospital pharmacy service to the Trust while constantly looking at ways to further improve our service.

We understand the importance of great team working and have strong links with other disciplines within and around the trust. The patient is at the Centre of all that we do, and we aim to provide a pharmacy service that has Medicine Optimisation as key.

SHPU has EPMA (electronic prescribing and medicines administrations) throughout the trust In-patient sites.
