Job summary

Cumbria, Northumberland, Tyne and Wear NHS Foundation Trust (CNTW) is seeking an enthusiastic clinical pharmacist to join our inpatient teams and provide clinical pharmacist support to community treatment teams across mental health specialties. This 2-year fixed term post is based at Morpeth locality sites.

You will develop clinical knowledge, work within a multidisciplinary team, and contribute to Medicines reconciliation, discharge planning, and holistic patient care, with extensive education

Qualifications

  • Registration with General Pharmaceutical Council (GPhC).
  • MPharm or equivalent degree in Pharmacy.
  • Evidence of CPD commitment per GPhC requirements.
  • Working towards postgraduate clinical qualification or portfolio pathway.

Responsibilities

  • Medicines reconciliation and review on admission and discharge.
  • Support patients and carers with management of medicines.
  • Optimise medicines with a holistic view of mental and physical health needs.
  • Discharge planning and liaison with primary care.
  • Work within standards, legislation and guidelines as a registered professional.
  • Act as a role model for staff and ensure professional conduct.

Job overview

We are looking for an enthusiastic patient-focussed clinical pharmacist looking to develop their clinical knowledge and skills working into our inpatient areas and providing clinical pharmacist support to our community treatment teams across a range of mental health specialities. This post is a 2 year fixed term position.

Based at locality sites in Morpeth.

Main duties of the job

Applicants should have excellent communication and team working skills and self-motivation to contribute to the development and delivery of the pharmacy service. We welcome applications from pharmacist colleagues working in all sectors of the profession. Prior working experience in mental health is not essential, as a comprehensive programme of education, training and clinical supervision will be provided. We will support you to provide high quality services to patients and healthcare professionals.

Across our Trust, pharmacy staff are fully integrated into and regarded as valued members of the multi-disciplinary team.

Our clinical pharmacy services include:

Medicines reconciliation and review on admission and discharge

Supporting patients and carers with the management of their medicines

Optimising medicines, taking a holistic view of both mental and physical healthcare needs

Physical health and wellbeing advice

Discharge planning and liaison with primary care

To personally work and encourage others to work within standards, legislation and guidelines in keeping with professional registration requirements

To act as a role model for other staff and to demonstrate personal conduct in a manner in keeping with being a registered professional.

Education and Qualification
  • Registration with General
  • MPharm or equivalent first degree in Pharmacy
  • Evidence of commitment to CPD as a minimum to meet the mandatory requirements of the General Pharmaceutical Council
  • Working towards postgraduate clinical qualification Or Evidence of portfolio working towards RPS post registration foundation pharmacist curriculum
Knowledge and Experience
  • Experience in providing clinical pharmacy services
  • Experience of hospital pharmacy
  • Experience of working within a mental healthcare setting
Skills and Competencies
  • Excellent communication skills with demonstrable ability to present complex or sensitive information to clinicians and senior pharmacists.
