Qualifications

  • Degree in Social Work, CQSW, DipSW or equivalent recognised Social Work qualification.
  • Evidence of continuing professional development relevant to adult and transition services.
  • Registration with Social Work England.
  • Post-qualification experience within Children’s Services, Adult Social Care, or Transition Services.
  • Experience supporting young people with disabilities, autism, learning disabilities, or complex needs.

Responsibilities

  • Undertake statutory assessments, reviews, and support planning for young people transitioning into Adult Social Care services.
  • Complete Care Act Assessments and contribute to EHCP reviews where appropriate.
  • Develop person-centred support plans that promote independence, wellbeing and positive life outcomes.
  • Work collaboratively with Children’s Services, Adult Social Care teams, health professionals, education providers, and voluntary sector organisations.
  • Manage a complex caseload, ensuring interventions are delivered within statutory timescales and professional standards.
