Wokingham Borough Council is seeking a Social Worker to join the Preparing for Adulthood Team within Adult Social Care. The role supports young people aged 14–25 transitioning to adulthood, coordinating education, employment, housing and health services, with multi-agency collaboration to ensure smooth progression.

Qualifications

• Registered Social Worker (SWE) with experience in social care or healthcare settings.
• Experience with SEND and complex needs is essential.
• Strong knowledge of Care Act, Mental Capacity Act and transition guidance.
• Ability to work in a multi-agency environment and communicate with families.

Responsibilities

• Work under the Care Act to assess and review needs of young people and carers.
• Maintain strengths-based, person-centred practice while promoting independence.
• Complete risk assessments and develop safeguarding plans as required.
• Collaborate with families, schools, health partners, and other agencies.
• Advocate for young people and ensure their voice is heard during transition.
