Job summary

North East Combined Authority is seeking a qualified social worker to join our Specialist and Lifelong Services as the Preparing for Adulthood and Transitions Lead. The role focuses on improving outcomes for young people preparing for adulthood, in collaboration with Children’s Services, Education and Commissioning.

You will work with the Head of Specialist and Lifelong Services, coordinate across teams, and provide person-centred social work support.

Qualifications

• Qualified social worker with experience in children’s or adult services.
• Experience delivering transition planning and knowledge of the Care Act.
• Ability to coordinate across Children’s Services, Education, Finance, Commissioning and Adult Social Care.
• Knowledge of Preparing for Adulthood and CHC.

Responsibilities

• Drive changes to practice to improve outcomes for young people preparing for adulthood.
• Coordinate between Children’s Services, Education, Finance, Commissioning and Adult Social Care.
• Identify gaps in provision and barriers to independent living.
• Undertake transition planning with young people and assess eligibility.
• Provide high-quality, person-centred social work support and coordinate Care Act assessments as needed.
• Support parents/carers with processes and financial assessments.
