Job summary

Wiltshire Council in Trowbridge is seeking an experienced Social Worker to join the Hospital Discharge Team, driving innovative discharge pathways and safe transfers of care. You will perform complex assessments and support safeguarding processes in a multi-disciplinary setting.

Based in West Wilts, you’ll apply the Mental Capacity Act 2005 and Care Act 2014, ensuring timely, person-centred decisions and effective coordination with partners across RUH and community services.

Qualifications

• Experience in hospital discharge planning and multidisciplinary collaboration.
• Ability to conduct complex assessments and contribute to safeguarding.
• Knowledge of the Mental Capacity Act 2005 and the Care Act 2014.
