We currently have Social Worker (Level 1) opportunities available in the following teams:

  • East ADAPT Community Mental Health Team - 17.5 hours per week
  • West ADAPT Community Mental Health Team – 35 hours per week
  • Older Adults Community Mental Health – 35 hours per week

About our teams

ADAPT East and West Teams

The ADAPT teams offer a flexible service to individuals presenting with complex needs (excluding Psychosis or Bipolar disorder) requiring secondary mental health care. The pathway provides focused support and therapeutic interventions for working-age adults aged 18 to 65. Referrals may come from the Greenwich Mental Health Hub via triage, inpatient wards, crisis teams, Early Intervention in Psychosis team, forensic services, or out-of-area transfers.

Older Adults Community Mental Health Team

This team is a specialist multidisciplinary service for adults over 65 experiencing severe mental health difficulties or with a dementia diagnosis. The team provides assessments, support, advice for carers, and interventions to promote independence and mental wellbeing.

The team supports individuals through care coordination, psychological groups, and collaboration with inpatient and crisis teams to assess and meet community needs, including discharge planning.

What we look for:

All social workers must manage caseloads and work co-productively with service users, families, and carers, maintaining a strength-based, person-centred approach.

They will work within a multidisciplinary team including social workers, occupational therapists, psychiatrists, psychologists, support workers, lived experience practitioners, community psychiatric nurses, and administrative staff.

Responsibilities include delivering assessment and care interventions that promote mental health and independence, and undertaking statutory functions such as:

  • Carers assessments
  • Court of protection
  • Safeguarding concerns and enquiries
  • Mental capacity assessments
  • Mental health assessments

All social workers must be willing to train as an Approved Mental Health Professional (AMHP) and act on behalf of the Royal Borough of Greenwich, depending on service needs.
