Job summary

Sanctuary Personnel in Lewisham, UK, is seeking a Social Worker to join the Learning Disabilities Team. The role offers flexible part-time or full-time hours with an hourly rate of £32.00, and a focus on improving outcomes for adults with learning disabilities, autism and complex needs.

You will conduct assessments, develop care plans, coordinate interventions, and work with carers and health partners to promote independence and wellbeing.

Responsibilities

• Conduct high-quality assessments and reviews, including annual reviews and progression reviews, to identify individual needs and aspirations.
• Develop and implement strengths-based, outcome-focused care and support plans that promote independence, resilience, and community inclusion.
• Coordinate timely interventions and support planning at key transition points.
• Work collaboratively with health professionals, carers, families, advocates, and partner agencies to deliver coordinated support.
• Support adults with learning disabilities, autism, and complex needs to achieve positive outcomes and reduce long-term dependency.
• Promote choice, control, independence, dignity, and wellbeing in all aspects of practice.
• Undertake Care Act assessments and reviews in accordance with relevant legislation and local procedures.
• Manage a varied caseload while maintaining accurate and timely case records.
• Participate in safeguarding processes and contribute to risk assessment and risk management activities.
• Engage in regular supervision, reflective practice, and continuous professional development.
