Job summary

Somerset Council is seeking a Social Worker in the Learning Disabilities team to support adults with learning disabilities and their carers to live independently and with choice. You will undertake assessments, manage risk, and develop personalised care and support plans under the Care Act 2014.

You will build meaningful relationships with individuals, families and carers while working in a collaborative, multi-agency environment in Somerset.

Responsibilities

• Undertake strengths-based assessments of need and risk for adults with learning disabilities and their carers.
• Manage a caseload of varying complexity with appropriate, proportionate care and regular reviews.
• Work directly with individuals and families to support informed decisions about care and support.
• Develop and maintain relationships with partner agencies including health, housing and voluntary sector organisations.
• Respond to safeguarding concerns, ensuring protection while promoting independence.
