Job description

We have an exciting opportunity for a Social Worker to join the Hospital Social Work Team

This is a full-time, permanent role with hybrid working, and a salary of £38,192 - £42,012 per annum plus an annual retention bonus of £3,000 subject to eligibility.

About The Team

The Hospital Social Work team works collaboratively with other healthcare professionals, such as doctors, nurses, and therapists, to ensure a comprehensive approach to care. We provide crucial psychosocial support to patients and their families, bridging the gap between medical care and social needs. Individual needs are assessed to develop discharge plans and connect patients with necessary resources and support in the community. Assessments focus on the whole person, not just the illness, to ensure a holistic approach to care.

Strength-based assessments and care planning focus on patients' social and emotional functioning, which includes family dynamics, financial situation, and housing.

The team works with patients, families, and other healthcare providers to develop a plan for safe and effective discharge, considering various options like home care, rehabilitation, or other facilities.

The team provides support with resources by connecting patients and families with relevant support services such as advocacy, financial assistance, social care, and housing.

The Role

As a Social Worker, you will be at the forefront of empowering adults to live independent and fulfilling lives. You will manage an allocated caseload, developing care plans and providing proportionate interventions in line with the Care Act. Your work will involve undertaking holistic, person-centred assessments and reviews to help residents achieve their personal goals and promote their independence. You’ll be responsible for responding to and raising safeguarding concerns, while also fulfilling key statutory duties under the Care Act, Mental Capacity Act, and Mental Health Act.

  • Qualified Social Worker with current registration with Social Work England.
  • BA, BSc or equivalent qualification.
  • Diploma qualification in Social Work.
  • Experience of working in a health or social care environment.
  • Strong understanding of relevant legislation, including the Care Act, Mental Capacity Act, and safeguarding protocols.
