Job summary

Rhondda Cynon Taf County Borough Council is seeking an agency social worker for Taf Ely Community Mental Health team in the Tonypandy/Pontypridd area. Role is office based at Dewi Sant Health Park, Pontypridd, with agile working available but no fixed home days.

The post is nine months, focusing on adults with mental health needs, with a multidisciplinary team including AMHPs, CPNs, psychiatrists, and more. Travel within the locality is essential.

Responsibilities

• Work as part of a team and understand the strengths and challenges of multi-disciplinary working.
• Safeguard and promote the wellbeing of adults through high-quality social work practice in assessment, care planning, and review.
• Co-produce and implement care and treatment plans and act as a care co-ordinator in accordance with key legislative requirements of the Mental Health Measures Wales (2010).
• Work within and understand the principles of the Social Services and Well-being (Wales) Act (2014).
• Undertake mental capacity assessments and work within the principles of the Mental Capacity Act 2005.
• Be familiar with WARRN risk assessments and assess risk in a collaborative and structured way that balances risk with autonomy and choice.
• Identify and promote individual strengths and independence.
• Work in collaboration with other agencies and adopt a multi-disciplinary approach where appropriate.
• Ensure that electronically held records, including case recordings, assessment, care and treatment plans, and reviews are completed and updated in accordance with departmental policy and procedures.

Job description

Salary: £22.20 per hour

The role will be office based in Dewi Sant Health Park, Pontypridd CF37 1LB. Agile working is available but due to the nature of service delivery it will not be possible to set aside dedicated agile working days.

We are looking for an agency social worker for a nine-month period to join the Taf Ely Community Mental health team. The team covers the Taf area of Rhondda Cynon Taf, roughly from Llantrisant to Pontypridd to Tafs Well.

The work will be focused on individuals with mental health needs. The team consists of a social work team manager, principal social workers, social workers (including AMHPs), community psychiatric nurses (CPN), nurse team lead, psychiatrists, psychologists, and an occupational therapist.

The expectation is that the role is site based 5 days a week, Monday to Friday. A 4-day week can be considered. There is scope to agile work from home, dependent on the needs of the service. We do not allow definite allocated days working from home because the team needs someone who is able to respond to the requirements of the team, attend the office, cover the duty desk, and undertake visits and face to face assessments.

Being able to travel independently in the locality is essential.
