What you will do:

• Manage a caseload of adults with complex social care needs, performing assessments and reviews in line with the Care Act 2014.
• Conduct safeguarding enquiries under Section 42 of the Care Act, ensuring risk assessments are carried out and protective planning is in place.
• Complete Mental Capacity Assessments and coordinate Best Interests decisions as required by the Mental Capacity Act 2005.
• Develop and review person-centred support plans that foster independence, wellbeing, and positive outcomes.
• Collaborate closely with NHS colleagues, Occupational Therapists, care providers, housing partners, and voluntary sector agencies.

Person Specification:

Qualifications:

• Degree or Master's Degree in Social Work or an equivalent recognised qualification.
• Successful completion of the Assessed and Supported Year in Employment (ASYE) is desirable.

Registration and Compliance:

• Registered with Social Work England.

Experience:

• Significant post-qualification experience within Adult Social Care.
• Experience managing complex community-based cases involving safeguarding concerns.
• Demonstrable experience conducting Mental Capacity Assessments and Best Interests processes.
