Job summary

Bolton Council is seeking an enthusiastic Social Worker to join Bolton’s Central North neighbourhood team, delivering person-centred support to adults and older people in the community. You will collaborate with a multi-disciplinary team to empower independence and better outcomes.

You will assess needs under Care Act, develop asset-based plans, and work within Mental Capacity Act and related legislation. DBS clearance, English fluency, and HCPC registration are required.

Qualifications

• Full driving license is essential.
• HCPC registration is essential.
• CQSW/DIPSW/SW degree or equivalent is essential.

Responsibilities

• Carry out Care Act 2014 assessments, including carers assessments.
• Develop asset-based, strengths-focused support plans.
• Apply Mental Capacity Act principles to practice.
• Work with health and social care partners to support outcomes.
