Sanctuary Personnel Limited is assisting Swindon Borough Council in recruiting a Social Worker for the Children with Disabilities (CWD) Team in Swindon.   • Undertake assessments of children and young people with disabilities and their families, identifying strengths, needs, and risks.
  • Develop, implement, and review child-centred support plans that promote positive outcomes.
  • Ensure the voice, wishes, feelings, and experiences of children and young people remain central to all decision-making.
  • Identify, assess, and respond effectively to safeguarding concerns.
  • Undertake statutory child protection work, including participation in strategy discussions, child protection conferences, and core group meetings.
  • Build positive relationships with children, families, carers, and support networks.
  • Work collaboratively with education, health professionals, and partner agencies to coordinate effective support.
  • Attend and contribute to multidisciplinary meetings, reviews, and planning processes.
