Qualifications

• Be a qualified Social Worker and registered with Social Work England.
• Knowledge of social work methodologies and relevant legislation including Care Act.
• Experience in assessment, risk management, safeguarding and court work.
• Experience with multi‑agency/multidisciplinary teams and hospital discharge processes.
• Experience with learning disability and autism is desirable.

Responsibilities

• Provide support to adults with physical disabilities and cognitive needs.
• Undertake Care Act assessments, safeguarding work and court involvement as required.
• Collaborate with partner agencies to identify and meet needs.
• Promote independence and wellbeing through outcomes‑focused practice.
• Participate in supervision, learning and development opportunities.
