Social Worker - Adults - Mental Health Transitions in Exeter UK, earning £31 to £35 per hour Hourly Full time

This is a full-time locum role offering £31 to £35 per hour depending on experience for a period of three months. This role is pivotal in guiding young individuals with mental health needs as they transition from children's to adult services. What you will do:

* Conduct comprehensive assessments to identify support needs and help develop community engagement and independent living skills.

* Work within Care Act principles to ensure young people can gain employment and live independently.

* Manage a varied caseload, maintaining up-to-date records of interventions and outcomes.
