Job Title: Social Worker – Adult Hospital Discharge – St Helens

Location: St Helens (Hybrid)

Pay Rate: £35 per hour

About the Role:

Liquid Personnel is recruiting a Qualified Social Worker for the Adult Hospital Discharge Team at St Helens. To be successful, you must be registered with the Social Work England (SWE) and hold a qualification equivalent to the Diploma in Social Work (DipSW).

Responsibilities:
  1. Complete assessments and related tasks to support hospital discharge at Whiston Hospital.
  2. Facilitate patient transitions to home or other care settings, ensuring appropriate support is in place.
  3. Operate effectively in a high-pressure, fast-paced environment, with experience in discharge planning preferred.
  4. Work full-time (37 hours per week) between 8:45 am and 5:15 pm, with a mix of office, remote, and ward-based work.
  5. Collaborate with multidisciplinary teams to ensure safe, person-centred discharge outcomes.
