Job Title: Social Worker – Adult Community Team

Location: Wakefield

Rate: £35 per hour

The Wakefield Council is seeking a qualified social worker with experience working with adults to join their team.

Responsibilities include:

  • Engaging in strengths-based conversations with service users, families, and advocates to determine eligibility and support planning.
  • Undertaking Capacity and Best Interest Assessments in line with the Mental Capacity Act 2005.
  • Conducting NHS Continuing Healthcare checklists and participating in Decision Support Tool (DST) meetings.
  • Managing a caseload effectively within a fast-paced environment.
  • Collaborating with a multi-disciplinary team, including health, pharmacy, housing, Age UK, and Carers Wakefield.

Qualifications and Experience:

  • Social Work England registration.
  • Eligibility to work in the UK.
  • Degree-level qualification in Social Work.
  • Post-qualified experience working with adults.
  • Understanding of the Care Act 2014 and the Mental Capacity Act 2005.
  • Experience in undertaking Capacity and Best Interest Assessments.
  • Familiarity with NHS Continuing Healthcare processes, including checklists and DST meetings.
  • Ability to manage a caseload in a fast-paced environment.
  • Strong communication, organizational, and interpersonal skills.
