Warwickshire County Council is seeking an experienced hospital-based social worker to provide a high-standard social work service for older adults and their families. You will manage caseloads, travel across Warwickshire, and collaborate with health partners in a fast-paced environment.
You will undertake robust assessments and care planning, work with a multidisciplinary team, and demonstrate a strong IT skillset while adhering to English language fluency requirements.
Joining our established Hospital Team, you'll be providing a social work service to a highly professional standard. Managing a caseload and travelling throughout the locality of North and South of Warwickshire depending with business need, you'll be working with older adults and their families and unpaid carers. Undertaking and recording robust assessments and care and support planning will also be part of your role. In addition, you'll be communicating with a wide variety of customers, communities, health partners and agencies as well as working on your own initiative, often in pressurised situations. Team members are supported by their colleagues and Team Managers who deliver regular supervision and day to day support.
