Social Worker- Adults within Community Team inTavistock, Devon earning £31 – 35 Hourly. What you will do:

– Undertake Care Act Assessments to assess and support the needs of adults in the community.
– Carry out Mental Capacity assessments and Best Interest outcomes, ensuring decisions are made in the best interests of the individuals involved.
– Complete Deprivation of Liberty Safeguards (DoLS) assessments with proficiency.
– Navigate Continuing Healthcare (CHC) checklists and Decision Support Tools (DSTs) as part of your assessment processes.
– Work from a Strengths-Based perspective and demonstrate Least Restrictive Practice in all interventions.
– Support the team duty function by triaging referrals and prioritising your daily case management activities effectively.
– Collaborate confidently with professional partners in a multidisciplinary team (MDT) approach.
